The RPi CM4 has USB 2.0, and it's perfectly possible to connect additional Ethernet. You can use the LAN7500 chip (a USB-to-Ethernet bridge) for this purpose. The Linux system will automatically detect the LAN7500 network card without any issues. However, the throughput will be limited to the maximum bandwidth provided by USB 2.0, which is around 280-300 Mbps in practice.CM4 only has one USB 2.0 OTG so not sure how that was usable for additional ethernet, maybe you mean Pi4 which has usb 3.0?
For creating a custom board, a good starting point can be the official CM5-IO board design files available at: https://pip.raspberrypi.com/categories/ ... sign-files
